CERTIFICATION

I understand that the uniforms that have been issued to me today are for my use during the 2021 season and I agree to return all uniforms within TWO (2) days of my final day of employment with Alaska Salmon Bake, Pick ‘n Poke Gift Shop or Fly Gal, Inc. In the event that I do not return the uniforms issued to me I understand that the cost of these uniforms will be deducted from my wages. In the event that I am not employed by Alaska Salmon Bake, Pick ‘n Poke Gift Shop or Fly Gal, Inc for a period of at least 30 days, or if I quit with no notice (no call/no show will be considered quitting with no notice), the company has the right to deduct a uniform set up fee of $50 from my final paycheck as well as the cost of any uniforms not returned.
